TemporoMandibular Joint Dysfunction, often called TMJ or TMD, is known as the “great imposter” because so many symptoms that patient complain of can be related to this disorder. Is it a migraine? In some circumstances we compensate it by using other muscles like jaw muscles and shoulder muscles. In rare cases neck pain can be caused by trauma. The most common treatment, called the Epley Manoeuvre, involves rotating the patient’s head and body through four specific movements at differing speeds, each held for about 30 seconds. The patient’s head and body are manipulated so that the dislodged crystals are moved along the canal and drop back into the correct chamber of the inner ear. The treatment is 98.4 per cent effective after three treatments.
Vestibular dysfunction caused by infection (labrynthitis, vestibular neuritis Meniere’s Disease), ototoxicity (from medication), barotraumas (such as explosion), vascular insufficiency or acoustic neuroma. Holding the head in the forward position to protect the jaw joint requires a lot of tension in the neck and back muscles. This constant tension also compresses the nerves that pass through these muscles, so some TMD patients also suffer from shoulder pain and even numbness or tingling in the fingers. In order to maintain balance and not fall forward, the body may also compensate by rotating the hips, resulting in lower back pain, and flaring the feet out, resulting in foot pain.
Various symptoms are associated with whiplash injury, including, but not limited to, neck and back pain, nausea, numbness, weakness, fatigue, dizziness, vertigo (loss of balance), inflammation, hearing impairment, headaches, visual disturbances, fracture, and paralysis. Occasionally, no symptoms are present initially, only showing up days, weeks, or even years after the injury. The consequences of whiplash range from mild pain for a few days (the most common outcome) to severe disability caused by restricted movement of the head or of the cervical spine, sometimes with persistent pain. Although a headache and dizziness are two separate and distinct conditions, there are many people who can experience them at the same time. The vestibular system includes the parts of the inner ear and brain that process sensory information involved with controlling balance and eye movements. If disease or injury damages these processing areas, vestibular disorders can result. The vestibular apparatus, a series of canals located inside the ear, includes a structure that contains tiny crystals of calcium carbonate. These crystals help the body detect gravity, tilting and speed changes in linear motions like stopping and starting at a traffic light. BPPV occurs when some of the crystals escape from their normal chamber and get into one or more of the canals. Vertigo symptoms are essentially caused by a confusion of signals to the brain.

Antidepressants are thought to be effective in treating pain because they increase the amount of neurotransmitters in the brain that decrease negative feelings like pain – both physically and mentally. Also, many people with chronic pain also experience depression, either due to the shared areas of the brain that process both physical and emotional pain or to the situational reduction of quality of life experienced by many with chronic pain. While using medication to target both conditions may seem sound, arguments against the use of antidepressants to treat chronic pain have been mounting for over a decade.
Treatment for whiplash includes passive and active exercises, gentle massage, chiropractic, therapeutic massage, spinal and extremity adjustments, as well as such therapies as traction to correct the spinal posture, electrical stimulation to relax the muscle spasms, stretching, and ice. All in all, this is actually great news. There are drug-based treatments such as using antihistamines and cardiovascular drugs, etc., but they can have negative side effects, and really only address the symptoms not the root cause. This type of vertigo is called Cervical Vertigo, or Cervicogenic Vertigo, and this is where we left off.
Vestibular system degeneration -With age, the function of the inner ear decreases, which can cause problems such as loss of balance, or disequilibrium. This can be complicated by mobility problems such as arthritis, resulting in falls. Good balance relies on three body systems working together: the eyes, the inner ear, and the joints (ankles, knees, hips and neck). Therefore, if one area is under performing, it can be helped by the other two. The causes of vertigo are either central, related to the brain or central nervous system, or peripheral, related to the organs of the inner ear. Central causes are associated with the brain itself such a tumor or stroke or are related to outside conditions which affect the brain indirectly like drugs and alcohol. Other disorders which affect the brain indirectly include heart disease and rhythm abnormalities which interrupt the supply of oxygen to the brain and can cause dizziness.
